The screening visit checklist provides space for you to list all relevant information that you may be asked during your screening visit, such as age at diagnosis, SMN2 copy number, medical and surgery history, and more. The handout also includes space to list details about your specific screening visit, such as date, time, and contact information, and suggested questions to ask.
